Trial Request Form

Filling out this form is the first step in hosting a trial

After the form is submitted, you will receive an email confirming receipt of your request. This is NOT an approval for your trial

Note: A trial request is required 6-12 months prior to the trial date to help the NACSW with trial planning.

The NACSW may request additional information to move your request through the approval process.
It is up to the host to determine the availability of a Certifying Official. We suggest you contact and try to schedule one for your trial event before or during the trial approval process. Upcoming dates that may affect CO availability: Aug. 14th - 16th, 2020, Oct. 13th - 21st, 2020. Dates will be updated as they become available.

The legal entity hosting the trial (individual, club, or business). For example: Jane Doe, or Jane's Dog Training, or Jane's Dog Training Club

Name of the person responsible for the pre-trial communication and responsibilities

Is the contact person performing the pre-trial communication and responsibilities the legal host, or acting as an agent on behalf of the host. The business owner or individual hosting is the legal host. Another person such as a staff member or club member would be an agent acting on behalf of the host.

Is this the first time you have hosted?

Is the trial request for the same trial levels and number of days as the COVID19 canceled trial?

Please fill in the formal name of location (for example: Kendall Middle School)

Please include full address including street address, city and state

Please be sure to list the month and year of ALL previous use of the trial site (not just the last trial).

Please list both start and end dates of your proposed trial. Trial dates that are 4 months or less from submission will be required to submit a CO Agreement and judges confirmation letters to before trial event will be approved.

May include classes, workshops, trials by other venues and any event that includes odor usage.

Please list the types (Element Specialty Trials and/or Nose Work Trials) and what levels (NW1, NW2)
PLEASE NOTE: Trial sites may not be re-used for NW3 or Elite trials for more than two years in a row. After two years of the same level, the site must be used for other levels or have a year off before re-use for the NW3 or Elite level. Summit Trial sites may only be used for one year then must have at least a year off before re-use.
