Trial Request Form

Filling out this form is the first step in hosting a trial

After the form is submitted, you will receive an email confirming receipt of your request. This is NOT an approval for your trial

Note: A trial request is required 6-12 months prior to the trial date to help the NACSW with trial planning.

The NACSW may request additional information to move your request through the approval process.
It is up to the host to determine the availability of a Certifying Official. We suggest you contact and try to schedule one for your trial event before or during the trial approval process. Upcoming dates that may affect CO availability: March 23rd/24th, 2019; April 27th/28th, 2019; August 31st/September 1st, 2019; Oct. 8th - 13th, 2019; Dec. 6th-7th, 2019. Dates will be updated as they become available.

The business name that the trial will be listed under.

This person will be the main contact for both the NACSW and the general public and will be listed on all announcements. They will be acting on behalf of the Trial Host. For example if the host is Wells Creek Dog Training then the agent would be Jean Richardson

Please fill in the formal name of location (for example: Kendall Middle School)

Please include full address including street address, city and state

Please be sure to list the month and year of ALL previous use of the trial site (not just the last trial).

Please list both start and end dates of your proposed trial.

Please list the types (Element Specialty Trials and/or Nose Work Trials) and what levels (NW1, NW2)

Please give a brief summary of your prior trial volunteer experience. Repeat trial hosts, please just put: "Experienced trial host".
